POSITION SUMMARY: Responsible for educating the community in hospice, state and federal regulations governing hospice, services provided, and generally the provision of hospice. A liaison between the community physician, client, and the agency in providing continuity of client care and the delivery of quality client care in hospice. Responsible for generating predetermined quota for Hospice admissions. ESSENTIAL JOB FUNCTIONS: 1. Identify, establish and maintain relationships with community referral sources. 2. Schedule and coordinate community in-services and education meetings which may include, but are not limited to: 1) Philosophy, mission & purpose of the hospice organization 2) Access to care 3) The Medicare Hospice Benefit 4) Advance Directives 5) Admission criteria and scope of services 6) Caregiver support and education 7) Bereavement services 8) Planning for end-of-life care
